Welcome to Yakima Valley Memorial Hospital!

We are pleased to have you be a part of our healthcare team! Below are forms, weblinks, training and information to help you be successful during your time with us. Please refer to your nursing program for additional information related to schedules and placements.

Required Hospital Forms
The following are forms required to PRIOR to your clinical rotation.

  1. HIPAA Education
  2. Workforce Member Confidentiality Agreement
  3. Information Systems Confidentiality Form

In addition, Yakima Valley Memorial Hospital also requires each nursing student to be current and have on file with your school:
• Current Washington State Patrol Clearance
• American Heart Association (AHA) CPR Certification for Health Professionals
• Professional liability insurance
• Health and accident insurance
• Tuberculosis screening results (annual)
• Evidence of immunization status for:

  • Hepatitis B
  • Mumps
  • Polio
  • Chickenpox/varicella
  • Influenza

Helping you become the BEST you can be in your transition to Yakima Valley Memorial Hospital

Extensive training opportunities are available to newly licensed staff or those who have been practicing patient care in settings other than hospital.

The orientation process at Yakima Valley Memorial Hospital is designed to fit your needs.

Hospital Orientation – 6 Days
• general orientation
• temperament training
• safety imperative
• constructive culture training

Unit Orientation – 6 weeks to 6 months
Preceptors and clinical educators develop an individual orientation for each new hire. This includes both unit education and ongoing support

An innovative, cutting edge program using Human and Medium Fidelity Patient Simulation. ACES prepares staff for optimal performance in both routine and unusual patient scenarios.

ACES combines discussion of key policy and procedures with hands on skills training and application. It is an opportunity for new staff to become familiar with memorial’s philosophies, policies, procedures, software, equipment and supplies.
